Slip disc treatment in Panchkula – back pain going down the leg

Back Pain That Travels Down the Leg

Most people who say "slip disc" mean one thing: a back problem that has started sending pain, tingling or numbness down one leg. The disc has not actually slipped out of place. Its soft centre has pushed through the outer ring and is pressing on a nerve root. Doctors call it a disc prolapse or herniated disc, and the lower back levels, L4-L5 and L5-S1, are where it happens most often.

Dr. Gaurav Sharma, Senior Consultant Neurosurgeon, sees slip disc patients every week at Advaith Brain & Spine Clinic in Sector 21 and at Paras Health - Panchkula in Sector 22. A good number of them have been told they need surgery. Many of them don't.

Do You Really Need an Operation?

For the majority of people, the answer is no. A disc that has burst out can shrink back over the following weeks as the body reabsorbs it. That is why the first plan is usually simple: pain relief, a short period of reduced activity (not complete bed rest), and physiotherapy once the worst pain eases. Many patients feel clearly better within six to twelve weeks.

The MRI often looks worse than the patient. A large disc on the scan does not automatically mean surgery, and a small one does not mean the pain is imaginary. The decision depends on what you are feeling and what the examination shows, not on the report alone.

Signs That Need Attention Sooner

Some symptoms change the plan. Please don't wait for an appointment slot if you notice:

  • difficulty passing urine, or losing control of the bladder or bowels
  • numbness around the private parts or inner thighs (the "saddle" area)
  • a foot that has started dragging, or weakness that is getting worse day by day
  • pain with fever, weight loss or a history of cancer

The first two can mean pressure on the bundle of nerves at the end of the spinal cord, called cauda equina syndrome. This needs an emergency MRI and often surgery within hours, so go straight to a hospital.

Treatment Options, Step by Step

Medicines and physiotherapy

Painkillers and nerve-pain medicines take the edge off so that you can keep moving. Walking is encouraged. A physiotherapist then works on core strength, posture and safe bending. Patients who skip this stage often find the pain returning a few months later.

Nerve root block

If leg pain is severe and not settling, an injection around the irritated nerve root can calm it down. It is done under imaging guidance and does not remove the disc, but it can buy time for natural healing and help people get back to physiotherapy.

Surgery

Surgery is discussed when pain stays severe despite proper treatment, when weakness is increasing, or in the emergencies listed above. The aim is to take out only the piece of disc that is pressing on the nerve. Depending on the disc's position and size, this is done by endoscopic (keyhole) discectomy or by microdiscectomy through a small cut using a microscope. Bigger operations such as fusion are rarely needed for a simple slip disc. If the spine is unstable as well, the options are explained on the minimally invasive spine surgery page.

After Disc Surgery

Most patients walk the same day or the next morning. Leg pain often eases quickly; numbness may take weeks to fade. Desk work is usually possible within two to four weeks, while heavy lifting and long two-wheeler rides are kept off for longer. Every patient gets a written plan for activity, driving and physiotherapy, because recovery is not identical for everyone.

Which Doctor Should You See for a Slip Disc?

Both neurosurgeons and orthopaedic spine surgeons treat disc problems. What matters is that the person examining you looks at the nerves carefully and is comfortable with non-surgical treatment as well as surgery. Bring your MRI films or CD, previous prescriptions and a list of medicines to the first visit.
